What are some ways to avoid adverse outcomes?
Asking ‘why?’, avoid blame, follow evidence based guidelines, continuity of care, maintain dr-pt relationship, self-care
What are some causes of iatrogenic disease?
impaired compliance, poor clinical assessment, polypharmacy, altered pharmacokinetics/dynamics of drugs with certain pathologies
What are some of the causes of non-compliance in patients?
not expecting benefit from treatment, running out of meds, lack of understanding of meds, NZ health system unfamiliar, failure to attend clinic due to bad experiences
What are some ways to improve compliance in patients?
Give simple instructions and provide rationale for treatment, clearly label medications, use pill boxes/blister packs, memory aids
What are the steps in assessing family/intimate partner violence?
- build relationship/trust
- ask/use direct questions
- acknowledge and validate abust
- document injuries
- assess risk of danger’
- make safety plan and empower patient to seek help
What are the benefits of mandatory reporting of domestic violence?
protects more victims, normalises reporting, validates victims concerns, statistics may be more accurate,
What are the disadvantages of mandatory reporting of domestic violence?
may damage dr-pt relationship, may cause more abuse, limits victims future healthcare trust, needs more resource-intensive systems in place, doctors may feel inadequately trained
What are some of the reasons an individual may find it difficult to leave an abusive partner?
6Fs - fatigue, fault, fear, finances, father, false beliefs
What are the aims of palliative care?
alleviate suffering, affirm life/accept death, family/firend support, improves quality of life, neither hastens/postpones death
