What is collaborative health care?
Different health care professionals come together to improve/restore a patient’s health.

Consultation vs Referral
Consultation: ED/hospitalist contacting a neurosurgeon to determine the best way to manage a patient with a brain bleed.
Referral: PCP puts in a request to have the GI see you for a specific GI complaint that they are unable to manage/figure out.
Are silos generally considered beneficial or detrimental in the health care setting?
Detrimental.
Lead to barriers in communication.
What is an example of a silo?
Us as nursing students. We do not have the opportunity to interact with other health care disciplines, therefore limiting our understanding of each others roles and scopes of practice.
Where do breakdowns in communication typically occur?
During handoffs and shift changes.
What tool is used to improve communication when handing off care to the next individual?
