What is endometriosis?

What is the pathophysiology of endometriosis?
Endometrial tissue is sensitive to oestrogen
Free blood from bleeding from the ectopic tissues during menstruation irritates and causes fibrosis, adhesions and infertility
What is the presentation of endometriosis?
Dysmenorrhoea (painful cyclical menstrual pain) Menorrhagia Dyspareunia Painful defecation Bloating/distension
What is seen on examination of endometriosis?
Fixed retroverted uterus
Reduced organ mobility
Tender nodularity in posterior vaginal fornix
What is seen on USS in endometriosis?
Chocolate cysts on ovaries
What is the diagnostic investigation for endometriosis?
Laparoscopy
What is the management of endometriosis?
Symptomatic relief - analgesia (NSAIDs)
1st - COCP/Mirena IUD
2nd - GnRH agonist
3rd - laparoscopic ablation
How does hormonal treatment help in endometriosis?
Hormonal therapy suppresses ovulation and allows foci to atrophy
What complications can endometriosis cause?
Subfertility
Pelvic adhesions
Ectopic pregnancy
