Empty appearing nuclei with orphan annie eyes, psammoma bodies, nuclear grooves
Lymphatic invasion common
Inc risk with RET and BRAF mutation, childhood irradiation

Good prognosis
Invades the capsule (unlike follicular adenoma)
Uniform follicles

Cancer if the parafollicular C cells - produces calcitonin
Sheets of cells in amyloid stroma
Hematogenous spread
Commmon in MEN2A and MEN2B (RET mutations)

Older pts
Invades local structures, very poor prognosis
Giant cells and biphasic spindles

associated with Hashimotos

Resection risks
A
- Hoarseness: due to nicking the recurrent laryngeal nerve
- Hypoccalcemia: due to removal of the PT glands
- Resection of the recurrent laryngeal nerve when removing the inferior thyroid artery
- resenction of the superior laryngeal nerve when removing the superior thyroid artery
