Drugs for managing BP in diabetes?

ACEi first line
UNLESS: pregnant or Afro-Caribbean
If pregnant - calcium channel blocker
if Afro-Caribbean - ACEi + (diuretic or CCB)

Blood glucose management drugs?

- Metformin (if no good, try drugs in 2)
- Add either:
a. DPP4i
b. piaglitazone
c. sulfonylurea - 1+a+c OR 1+b+c
- Insulin
OR
Metformin and sulfonylurea and GLP1 mimetic

Sulfonylureas?
A
Gliclazide, tolbutamide, glimepiride
Weight gain
Need some pancreatic function as augment insulin secretion

Gliptins
Can aid weight loss

Contraindicated in heart failure and bladder cancer, uninvestigated haematuria
