Accepting in offer ends an offer. This must be communicated, must match terms of the offer and must be certain.

The longer the time between the offer and acceptance, the more chance there is that the offer will be seen to have lapsed.

Offeree dies - offer ends.
Offeror dies - offeree can accept until offeree learns of offerors death.
