with septic shock?
Vancomycin 25mg/kg IV (max dose is 2g)
plus
Cefepime 2g IV
plus
Azithromycin 500mg IV
Abx used for UROSEPSIS?
Ceftriaxone 1g IV
OR
Ciprofloxacin 400mg IV
Consider + Vancomycin 25mg/kg if history of MRSA or Enterococcus infection.
Abx for UROSEPSIS w/septic shock:
Cefepime 2g IV
OR
Meropenem 1g IV
Consider + Vancomycin 25mg/kg if history of MRSA or Enterococcus infection.
Abx for severe and NON-purulent skin or soft tissue infection:
Vancomycin 25mg/kg IV
plus
Piperacillin/Tazobactam 4.5g IV
OR
Meropenem 1g IV
Consider surgical consultation
Severe and PURULENT
Incision and drainage
plus
Vancomycin 25mg/kg
plus
Surgical consultation
Abx given with unknown source or concern for intra-abdominal source:
Vancomycin 25mg/kg
plus
Piperacillin/ Tazobactam 4.5g

Meropenem 1g
