A gentleman has presented to A&E with possible sepsis 2 days after a TRUS biopsy. What is sepsis?

A

Life-threatening organ dysfunction due to a dysregulated host response to infection.
Septic shock is defined as a subset of sepsis in which particularly profound circulatory, cellular, and metabolic abnormalities substantially increase mortality.

How do you assess for sepsis?

A

qSOFA [Quick Sequential Organ Failure Assessment] can be used to assess for organ dysfunction
This involves 3 tests to assess for risk of sepsis
Altered mental status, RR >22, Systolic BP of <100mmHg
2 or more of these increase the risk of prolonged ICU stay or death

A gentleman has presented to A&E with possible sepsis 2 days after a TRUS biopsy. How would you proceed?

A

I would immediate review the patient and assess him in a ABCDE approached according to the ATLS algorithm.
If not already done so by the A&E team I would start the sepsis 6 management which would be high flow O2, IV access and fluids, IV abx, Blood cultures, lactate and monitoring urine output via catheter.
Antibiotic regimen would be in accordance to local guidelines which my hospital case would be Amoxicillin and gentamicin, I would clarify allergies prior to giving this.
I would admit the patient for antibiotics and observe but if there is no improvement I would arrange a CTAP for possible abscess
