Stable svt, narrow complex tachycardia
Adenosine 6 mg Iv rapid push
12 mg in 2 min if no effect
Pulseless VF/VT
Amiodarone 300 mg ivp
Followed by 150 mg ivp at next pulse check
Stable wide complex tachycardia
Amiodarone 150 mg IV over 10 min
Followed by 1mg/min x 6 hrs
Followed by 0.5 mg/ min thereafter
Anaphylaxis
0.1-0.5 mg 1:1000 IM
0.3 max single dose!
Rapid sequence intubation
Etomidate 0.3mg/kg
Usually 20 mg IV
Heparin reversal
Protamine sulfate 1mg neutralizes 100 units
5mg/min
Bowel motility/ nausea
Metoclopramide 10 mg IV
Hypotension refractory to IVF
Levophed 1-30 mcg/min IV
Bleeding esophageal varices
Octreotide 50 mcg IV bolus + 50 mcg/ hr IV
Nausea
Ondansetron (zofran) 4 -8 mg IV Q4-Q6 prn
Metabolic acidosis
50 meq of sodium bicarbonate
Beta blocker reversal
Glucagon 3-10 mg IV loading dose
Torsades
Magnesium sulfate 2g IV push
NSTEMI anticoagulation
Lovenox 1 mg/kg q12 hrs
Stable afib with RVR/ Stable SVT
Cardizem 0.25 mg/kg IV x1
5-15 mg/hr after
