Bradycardia treatment

A

Stable:

Observe

Unstable or risk of asystole (i.e. Mobitz Type 2, complete heart block or previous asystole):

1st line:

Atropine 500mcg IV

No improvement:

  • Atropine 500mcg IV repeated (up to 6 doses for a total to 3mg)
  • Other inotropes (ex noradrenaline/isoprenaline)
  • Transcutaneous cardiac pacing (using a defibrillator)

Acute Mx of Stable patients with SVT

A

Make sure they are on continuous ECG monitoring!

  1. Valsalva manoeuvre.
  2. Carotid sinus massage
  3. Adenosine
  4. An alternative to adenosine is verapamil (calcium channel blocker)
  5. Direct current cardioversion may be required if the above treatment fails
