Describe ECG changes in hypokalaemia
T wave flattening, U waves, prolonged PR
What is the management for hypokalaemia?
If potassium > 3 and no ECG changes - Sando K
If potassium < 3 or ECG chanegs - IV potassium with cardiac monitoring
Describe ECG changes in hyperkalaemia
Peaked T waves, wide QRS
What is the management of hyperkalaemia?
IV calcium gluconate, IV insulin and dextrose
What is the management of hyponatraemia?
Hypovolaemic - fluids
Euvolaemic - fluid restriction
Hypervolaemic - diuretics
Severe - hypertonic saline in ICU
What is the management of hypernatraemia?
Gentle rehydration with hypotonic fluids eg dextrose, or if hypovolaemic give IV fluids
What can happen if hyponatraemia is corrected too quickly?
central pontine myelinosis
What can happen if hypernatraemia is corrected too quickly?
Cerebral oedema
Describe ECG changes in hypocalcaemia
Prolonged QT -> Torsades de pointes
What is the management of hypocalcaemia?
Mild - oral calcium carbonate
ECG changes - IV calcium gluconate
Describe ECG changes in hypercalcaemia
Shortening of QT -> AV node block
What is the management of hypercalcaemia?
Aggressive fluid resuscitation
If severe consider IV pamidronate
What is the significance of hypomagnesaemia?
Often accompanies hypokalaemia, replace magnesium first !!!
What is the management of hypophosphataemia?
Oral phosphate
What is the management of hyperphosophataemia?
Senior management renal replacement therapy
