Elective Surgery Flashcards
Pseudotumour
Metal particles cause inflammatory granuloma = muscle and bone necrosis
Early local complications
Infection, dislocation, instability, fracture, leg length discrepancy, nerve injury, bleeding, DVT
Early general complications
Hypovolaemia, shock, AKI, MI, ARDS, PE, UTI,
Late local complications
Infection (blood), loosening, fracture, pseudotumour
Which antibiotics predispose to tendon rupture?
Quinolones, e.g. ciprofloxacin
Treatment of tendon tear/rupture
Rest, analgesia, NSAIDs
Steroids (RC, elbow)
Synovectomy, decompression
Splintage, surgical repair
Treatment of instability
Soft tissue procedures - ligament tightening/reconstruction
Bony procedure - fusion, osteotomy
Treatment of contractures
Splints, physio, botox/baclofen
Surgery - fasciotomy, fasciectomy, tendon transfer
Osteomyelitis
Infection of bone
Trauma/surgery/haematogenous
Osteolysis, pus, involucrum, sequestrum
Sequestrum
Dead fragement of bone in osteomyelitis = need more than antibiotics
Involucrum
New bone around area of necrosis in osteomyelitis
Acute osteomyelitis
Kids and immcomp
Sluggish tortutous BVs = accumulation of bacteria
Loose periosteum = subperiosteal spread
Subacute osteomyelitis
More insidious
Bone walls off abscess with sclerotic bone (Brodie’s abscess)
Treatment of acute osteomyelitis
IV antibiotics
Drainage
Surgery
Chronic osteomyelitis
Untreated acute osteomyelitis
Sequestrum, involucrum
Axial skeleton
