Elbow/Wrist/Hand Flashcards
What is the elbow extension test?
Test to determine if radiographs are necessary following a traumatic elbow injury
If patient can fully extend their elbow following an injury, radiographs do not need to be ordered right away
What are the imaging rules for the wrist?
There are none
instead use good clinical reasoning and look for
-scaphoid tenderness
-traumatic history
-willingness to move the wrist
-exquisite pain on palpation
What is Kienbock’s Disease?
What is Presier’s Disease?
avascular necrosis of the lunate
avascular necrosis of the scaphoid
What is skier’s thumb?
A tear of the ulnar collateral ligament at the MCP
If someone suffered an injury at their TFCC where might their pain be located?
How is this injury typical acquired?
Ulnar side of the proximal wrist
FOOSH injury where wrist is pronated and compressed
What is the ulnar fovea sign?
A test to determine ulnar sided wrist pain where examiner applies pressure to the ulnar fovea and if there is TTP test is (+) (High specificity and sensitivity)
Helps determine if pain due to TFCC
What is a ganglion cyst of the wrist?
a palpable cyst located on the base of the hand at the wrist which may be painful during movement of the wrist
What is Mallet finger?
What is sweater finger?
What is trigger finger?
A rupture or avulsion of the extensor tendon leading to inability to extend distal phalanx
Inability to flex distal phalanx
thickening of the flexor tendon sheath leading to difficulty extending finger (most common in 4th and 5th digits)
What is a boutonniere’s deformity?
What is a swan neck deformity?
a medical condition in which the finger is flexed at the proximal interphalangeal joint (PIP) and hyperextended at the distal interphalangeal joint (DIP) caused by rupture or central tendon slip of the extensor hood
a bending in (flexion) of the base of the finger, a straightening out (extension) of the middle joint, and a bending in (flexion) of the outermost joint due to a volar plate tear in PIP with an intrinsic contracture
What is a Dupuytren’s Contracture?
flexion of the last two digits due to knots of tissue formed under the skin
What is a Boxer’s Fracture?
a break in the neck of the 5th metacarpal bone in the hand. It usually happens when you punch an object at a high speed. Symptoms of a boxer’s fracture include pain and swelling of the hand, limited range of motion of the pinky finger, and misalignment of the finger
What is the most common way of treating finger deformities?
Splinting
What is a Volar Plate?
A thick ligament on the underside of the middle finger joint. It keeps the finger from hyperextending back. Collateral ligaments – 2 ligaments, 1 on each side of the joint, that control how the finger bends and straightens. They prevent the finger from hyperextending sideways.
What is the moving valgus stress test?
A test to determine if there was an injury to the medial collateral ligament of the elbow where the examiner applies a valgus force on a fully flexed elbow and then extends elbow to determine if pain is reproduced
What are the big 3 tests for lateral epicondylagia?
Mills-Passive flexion and pronation test
Maudsley’s-Resist third digit extension-high sensitivity
Cozens-resisted elbow extension test-very sensitive not very specific
