When should a Hazard Assessment be completed?
When a hazard still remains in the system.
Ex., Liquid present in a vessel even when the vessel has been RTM’d (red tagged).
Ex., If chemicals are present in the equipment/piping (considered in-service), or utilities are operational on the equipment. Includes temporarily connected energy sources like nitrogen hose.

- Confined space entry

- Erecting, modifying, or removing temporary Life-Critical Guards used for fall prevention
*What is a Life-Critical Guard?

Imminent danger areas. May ONLY be entered by authorized personnel (like people working to eliminate the problem)
White warning tape identifies what? What must be worn in the area?
Areas that contain asbestos hazards. May not be entered without approved PPE.
What does magenta and yellow tape identify?
Radiation hazards. May only be entered by authorized personnel.
What does yellow & black “caution” tape identify?
When can the area be entered?
Non-imminent danger hazards.
May be entered after recognizing the hazard and proceeding with caution.
When is a Fall Prevention Plan (with continuous fall prevention) required?
For walking on elevated pipelines/cable trays/etc. where permanent fall prevention systems aren’t in place. Required Facility Leadership approval. Cable tray walking also requires a SWP.
