When should a Hazard Assessment be completed?

A

When a hazard still remains in the system.
Ex., Liquid present in a vessel even when the vessel has been RTM’d (red tagged).
Ex., If chemicals are present in the equipment/piping (considered in-service), or utilities are operational on the equipment. Includes temporarily connected energy sources like nitrogen hose.

A

SSI (Safety System Impairment) tag. If valve is modified/closed, a safety device won’t work as intended. For valves, used with carseals. Can be used on blowback spool on a SIS instrument.

A

A tag used like a lock. Means DO NOT OPERATE. One plus means 1 = intended valve position, + = securing from inadvertent operation.

  4. What alternate mitigations will be in place during impairment (for SSIS) & how they’ll be managed
