What is the recommended dose of edoxaban?
A
60mg OD

When would you reduce the dose to 30mg OD?
A
If one or more of the following are present:
- Moderate or severe renal impairment (creatinine clearance [CrCL] 15–50 mL/min).
- Body weight of 60 kg or less.
- The person is receiving concomitant treatment with any of following P-glycoprotein (P-gp) inhibitors: ciclosporin, dronedarone, erythromycin, or ketoconazole.

How would you switch from edoxaban to warfarin?
A
- For people taking 60 mg of edoxaban, prescribe 30 mg once daily with warfarin.
- For people taking 30 mg of edoxaban, prescribe 15 mg once daily with warfarin.

What would you monitor prior to starting edoxaban?
A
- Body weight
- Renal function (U&Es, calculated CrCl)
- Baseline Clotting Screen
- Full blood count
- LFTs
- BP

What would ongoing monitoring for edoxaban be?
A
- U&Es, CrCl, LFTs, FBC
6 monthly for people over 75

A
- can be taken up to 12 hours prior to when next dose is due
