what is eczema
it is skin that is itching, red and sometimes scaly
one can tell it is eczema because it is lacking a sharp margin
the most common for is atopic dermatitis
presentation of eczema
thickened discolored lesions
less demarcated
Itchy
papules
scalling
redness
crusting
Lichenefication
more likely to fissure if chronic
atopic dermatitis causes
multifactorial
genetic - atopic triad
infection
allergy
Diet
if in children usually outgrow it by teen years
treating atopic dermatitis
avoid allergens
emollients even when eczema is less active it helps hydration and acts as a barrier
bath emollients
soap substitutes
if severe use greasy emollients
steroids depend on site age and severity
1% hyrocortisone topical safe on face flexures and groin
betamethasone 0.1% higher potency on other location and reduce slowly
clobetasol is very potent only use briefly on thick skin
topical immunomodilators
phototherapy - UVB and UVA
systemic steroids
immunosuppressants
azathiprine
ciclosporine
What is seborrheic dermatitis
red scaly rash affects
greasy yellowish scales
Lichenefication - thickening and leathery
scalp eyebrows nasolabial folds cheeks flexures
caused by overgrowth of skin yeast
can be bad in HIV
treat with topical steroids/antifungals
Discoid eczema
disc shaped obvs
well demarcated plaques
crusted
Vericose eczema
on vericose veins
usually obese –> verivose veins
Pompholyx
affects palms and soles
usually triggered by heat or emotion
clear itchy vesicles
asteatotic dermatitis
elderly
diuretics
Exogenous eczema irritant contact dermatitis
very localised
caused by exposure to irritants like
cement
soap
oils
Exogenous eczema allergic contact dermatitis
commonly caused by
nickel
hair dye
rubber
