Genetic
Abnormality of fillagrin expression- filament associated protein which binds to keratin fibrs in epidermal cells. Inflamation due to inherited abnormality in “barrier defect” increased permeabilty and reduces antimicrobial function
Classification
endogenous
- internal trigger
- Atopic
- seborrhoeic
- dicoid
- varicose
- pomyphloxy
Exogenous
- contact (allergic, irritant)
- photoreactant (allergic, drug)
Atopic eczema
Itchy!!!
- 10-15% of infants, remission in 75%
- 2/3rds family history of atopy
- associated atopy- asthma, hayfeverer, conjunctivitis
- high IgE
infant atopic eczema
- itchy
- facial component
- occasionally vesicular
- secondary infection
- <50% still have eczema by 18 month
Management of atopic dermatitis
- emollients - moisture and replacement of skin oil barrier
- topical steroids - redness and itch
- bandages - keeps skin treatment in place
- anti-histamines (sedating ones)- itch
- Abx - swab first
Complications of atopic eczema
Admitt the following
- Bacteral infection - staph aureus
- viral - molluscum, viral warts, ecxema herpticum - Painful - acyclovir
Tiredness, growth reduction, psychological impact
Contact dermatitis definition
- precipitated by an exogenous agent
- type 4 delayed T cell
types of contact dermatitis
- irritant - direct noxious effect on skin
- allergic - type IV hypersensitivity
Common allergens in contact dermatitis
- nickel
- chromate
- cobalt
- fragrance
Seborrhoeic dermatitis
Chronic, scaly inflammatory condition often throught to be dandruff that is worse in teenagers
where do you get seborrhoeic dermatitis?
- face
- scalp
- eyebrows
- occ upper chest
Management of seborrhoeic dermatitis
Scalp - medicated anti yeast shampoo
face- anti-microbial, mild steroid
UV sunlight
simple moisturiser
Venous dermatitis definition
Underlying venouos disease that affects the lower legs due to an incompetence of deep performating veins that increases hydrostatic presure
management of venous dermatitis
- Emollients
- mild/moderate topical steroid
- compression bandaging/stockings
- consider early venous surgical intervention
