Indications for ECMO in Severe ARDS?
- Severe Hypoxemia (PaO2 to FiO2 ratio <80 despite high levels of peep (15-20 cmH20) for at least 6 hours in patients with potentially reversible respiratory failure.
- Uncompensated Hypercapnia with acidemia (pH < 7.15) despite the best accepted standard of care for management with a ventilator.
- Excessively high end-inspiratory plateau pressure (> 35-40 cmH2O, according to the patients body size) despite the best accepted standard of care for management with a ventilator.
Absolute contraindication for ECMO in setting of Severe ARDS?
Any condition that preludes the use of Anticoagulation therapy
Which ECMO can you use in setting of Right Heart Failure? (Venoarterial or Venovenous)
- Venoarterial ECMO (VA-ECMO) provides Cardiac Support bypasses pulmonary Circulation and decreases pulmonary artery pressure.
- Can be used in Right Heart Failure
- Achieves Higher PaO2
- VV-ECMO does not provide cardiac support and only maintains pulmonary blood flow (therefore cannot use with Right heart Failure)
