ECG conference 3/20/14 Flashcards
6 week old FTT sweaty when feeding
How do ALCAPAs tend to present
If they have infarcted - could present in failure or shock or they could present with the worst reflux and will posture and writhe - You would expect them to lose weight
When do ALCAPAs present and why
4-6 weeks old (at around this time pulm vasc resistance goes down) steal of blood into pulm vasculature
True or false can you have absent murmur in large VSD
True - AV canal may present without a murmur
TAPVR how would this present
Not many symptoms if not obstructed
PAPVR - physiology
Similar to ASD
EKG major factor to look for to diagnose cong heart disease
axis axis axis
What sort of axis do you get with AVC
North west axis
Changes in ALCAPA
Q wave in lead 1 and AVL v4-6 ( anywhere that shows lateral ischemia)
T wave inversion in lead 1 AVL and V4-6
What is the reason for a murmur in ALCAPA
Can be due to MR ( due to infarcted papillary muscle) - systolic but regurgitant - may be due to dilation secondary to ischemia
May hear pericardial friction rub if infarct
Q wave abnormality - how would you identify in ALCAPA
More than one box wide and 3 mm deep
When should T waves invert
7 days to 7 years
PS murmur
Would be a long murmur possible click
Coarcts will present with RVH true or false
True - because RV is experiencing the obstruction in utero - only in older child would you get LVH
Ebsteins anomaly - what is the axis
R axis - R A enlargement with large P waves
Other condition that give you NW axis
partial canal that is not an ASD
