I +ve, II -ve = leaving i.e. LAD -30 to -90 degrees
I -ve, II +ve = reaching i.e. RAD +90 to +180 degrees
Causes of RAD
Anterolateral MI
RVH
PE
Causes of LAD
Inferior MI
LVH
ASD - primum lower and larger
Causes of abnormal P waves
Absent: AF, SAN block, nodal rhythm
Dissociated: complete heart block
P.mitrale (LA hypertrophy->bifid waves): HTN, AS, MR, MS
P.pulmonale (RA hypertrophy->peaked waves): pulm HTN, COPD
Causes of abnormal QRS complexes
Wide QRS >120 ms: ventric. initiation, conduction defect, WPW
Pathological Q wave >1mm wide >2mm deep: full thickness MI
