What happens in anorexia Nervosa?

Also genetic predisposition: monozygotic twins
Higher social classes high prevalence
Aetiology:
Hormonal: down sex hormones
Psychological: family stress. Escapism from normal life, reggression into childhood,.. Perfectionism and low esteem antecedents.
Survivors of childhood sexual abuse high risk.
Treatment: outpatients if ok
How do you diagnose anorexia nervosa?
All of following:
BMI
Sx of anorexia nervosa?
Sensitivity to cold, constipation, hypotension and bradycardia.
If binge and vomit or abuse purgatives : hypokalaemia and alkalosis.
Tx of anorexia nervosa?
Outpatients if stable or if CVS sx admit and feed via NG tube, .CBT and family therapy session.
What happens in Bulimia Nervosa?
Binges: episodes of uncontrollable excessive eating
Accompined by efforts to lose weight.
High school girls. Unis, collages,
Premorbid dieting hx, better prognosis than in anorexia.
Behaviours: self induced vomiting
Misuese of drugs such as laxatives, diuretics, thyroid exract, anorectics.
How do u dx bulimia nervosa?
All of following:
binge eating
Methods ro prevent gaining weight (laxatives, vomiting, purging- in absence of eating episodes)
Morbid dread of fatness
What is an effective tx in Bulimia?
Individual CBT far more effective than drufs or psychotherapy

SSRIs can be effective even in the absence of a depressive illness but may require high doses.
