Anorexia Nervosa DSM-5 Criteria
Restriction of energy intake relative to calorie requirements
Also fear of weight gain & severe body image disturbances with body image the measure of self-worth
Restricting - restrick intake
Binge eating/purging: anorexic if 15% below IBW
Bulimia DSM-5 Criteria
A. Recurrent binge eating with sense of lack of control
B. Recurrent inappropriate compensation to prevent gain
-vomiting, laxative, diuretics, fasting, exercise
C. Occurs once a week for 3 months
D. Self-evaluation unduly influenced by shape and weight
E. Disturbance doesn’t occur exclusively w/ anorexic periods
Eating Disorder NOS DSM-5 Criteria
- All anorexia criteria w/ regular menses
- all anorexia criteria w/ normal weight
- all bulimia criteria except binges <2X/week or <3 months
- Normal BW with inappropriate compensatory behavior
- Pt chews and spits out large amounts w/o swallowing
Binge Eating Disorders Criteria
3 out of 5 Criteria:
- eating more rapidly than normal
- eating until uncomfortably full
- eating large amounts when not feeling hungry
- eat alone due to embarrassment
- feel disgusted, depressed, or very guilty w/ overeating
2 days a week for 6 months
SCOFF
Make yourself Sick when full
Lost Control over how much you eat
Lost >14 lbs (One stone) in 3 months
Think you’re Fat when others say you’re too thin
Food dominates your life
Anorexia Treatment
CBT - limited efficacy
Meds - disappointing results, really only work on co-morbid disorders
- anxiolytics, Zyprexa
Nutrition - regain 90-95% IBW is goal
Anorexia Hospitalization Criteria
<75% IBW
Electrolytes
Cardiac
Unstable pt
Failed outpatient treatment
Bulimia Treatment
CBT is effective
High success rate w/ drugs
- Fluoxetine (SSRI)
- TCA
- Topiramate
- Ondansetron

Pharmacotherapy
Female Athlete Triad
Eating disorders
Stress fractures
Amenorrhea
Tx: Estrogen (3 yr post-menarche of hx stress fx), decreased energy expenditure, nutrition, calcium w/ Vitamin D
Avoid NSAIDs - delay bone healing
