A

o Swelling/lump is noticed in the midline of the anterior neck

o Usually asymptomatic

o May be tenderness or rapid enlargement due to infection

o Patient being around the age of 5

WHat are the signs of thyroglossal cysts on examination?

A

o Midline, smooth, rounded swelling - typically found between the thyroid notch and the hyoid bone

o Moves upwards on protrusion of the tongue and with swallowing

o Can be transilluminated

What are the appropriate investigations for thyroglossal cysts?

A

o None may be necessary if the patient is euthyroid

o If the cyst is suprahyoid, TFTs should be performed to exclude a lingual thyroid - because removal of the lingual thyroid will make the patient hypothyroid

o Ultrasound/MRI - helps differentiate from other structures

A

o Pain in the throat - may be reffered to the ears or abdomen (in small children)

o Painful swallowing

o Headache

o Loss of voice or changes in voice

What are the signs of tonsillitis on examination?

A

o Red throat

o Swollen tonsils, which may have white flecks of pus

o High temperature (sometimes)

o Swollen lymph nodes

o Classic streptococcal tonsilitis features = acute onset, headache, abdominal pain, dysphagia, intense erythema of tonsils and pharynx, yellow exudate, tender, enlarged anterior cervical glands

WHat are the appropriate investigations for tonsillitis?

A

o Throat swabs and rapid antigen tests can be performed (but are NOT recommended)

o Swabs may not be able to distinguish between infection and colonisation
