Dysphagia Flashcards
Where does the oesophagus begin?
inferior pharyngeal constrictor merges with the cricopharygeus
How long is an oesophagus?
18-26cm
Where are there indentations in the oesophagus?
aortic arch
diaphragmatic hiatus
anterior body of C7
What causes odynophagia?
infection
tablets
If there is a problem with primarily solids, what tends to be the reason?
structural lesion like a malignancy or peptic stricture
If there is a problem with swallowing both solids and liquids what is more likely?
motility disorder like achalasia or scleroderma
What are the investigations carried out in dysphagia?
OGD and biopsy
What medications can cause dysphagia?
antipsychotics (dopamine blockers) tricyclic antidepressants (anticholinergic) opioids (increase lower oesophageal contractility) calcium channel blockers (smooth muscle relaxation)
What is the normal lumen of the oesophagus?
20mm
When does dyspahgia become a problem when the lumen of the oespophagus is smaller?
<15mm
What are some benign strictures causing dysphagia?
schatki rings caustic strictures peptic strictures anastomotic radiation induced oesophageal webs
What is achalasia?
insufficient LOS relaxation
loss of oesophageal peristalsis
Why can achalasia occur?
loss of ganglion cells in the myenteric plexus
infiltrate of T lymphocytes, eosinophils and mast cells
What are the symtpoms of achalasia?
dysphagia of solids and liquids
regurgitation
chest pain
How is achalasia diagnosed?
barium oesophagram with fluroscopy
