Fomepizole (antizol)
Works by inhibiting ADH in the cases of overdose due to antifreeze or MeOH poisoning and it will slow down the formation of Formaldehyde which is a toxic chemical
Disulfiram (Antabuse)
Irreversible inhibitor of ALDH which means that if a patient were to consume alcohol while on this it would be metabolized to acetaldehyde which causes you to get sick and vomit
used as aversive therapy
effects of the drug last for up to 14 days so even if patient misses a day they will still get sick if they consume alcohol
Alcohols action
Main target is GABAa receptor - it is allosteric activator of inhibitory neurotransmitters which means it allows more chloride into the cell making it more negative
legal limit
0.8%
Side effects of alcohol
Euphoria, talkative, analgesia, aggression, mood swings, CNS depression at high levels
Chronic effects of alcohol
cardiomyopathy, arrhythmia, hypertension, hemostasis
Drug interactions
CNS depressants
Aldehyde dehydrogenase inhibitors
Acetaminophen
Aspirin
Acamprosate (Campral)
NMDA receptor antagonist/ GABA agonist
- some of the same activity of alcohol (replacement therapy)
Naltrexone (Revia)
Opioid receptor antagonists
if people drink while taking this medication there is not as much as a reward feeling towards drinking
Mutation in Mu receptor
SNP mutation that allows enkephalins to bind more tightly to the receptor and you get more rewarding effects
Patients with this mutation respond better to Naltrexone
Baclofen (Baclofen)
good for patients who are having anxiety related to withdrawal of alcohol
