Thrombolysis AMI

A
  1. Aspirin
  2. Clexane
    Give enoxaparin between 15 minutes before and 30 minutes after the start of treatment. Treat for up to 8 days.

<75 years, IV 30 mg bolus plus SC 1 mg/kg, then SC 1 mg/kg every 12 hours (maximum 100 mg for the first 2 SC doses).

> 75 years, CrCl <30 mL/minute, SC 1 mg/kg once daily (maximum 75 mg for the first dose).

  1. Thrombolytic agent
    Tenectaplase
    0.5 mg/kg up to 50mg
    Consider half dose for > 75 years

Thrombolysis PE

A

Alteplase
IV, bolus of 10 mg, then 90 mg infused over 2 hours
(if <65 kg total dose = 1.5 mg/kg).

Cardiac arrest 50mg IV, then rpt 15 mins later

Tenectaplase
0.5 mg/kg up to 50mg
Consider 0.25mg/kg for >75 years
Same dose for cardiac arrest
