Substernal chest pain that radiates to the shoulder an hour and half ago while he
Was helping his son moving his furniture. Medical history of diabetes mellitus, hypertension and hypercholesterolemia. He is a non-smoker. His vitals are as follows: BP: 138/94, HR: 95, SpO2: 96%, Temp: 98 ℉. He is 71” tall and he weighs 220 Lbs. Family history revealed that his father had died of heart attack at age 67.
Is It a Heart Attack?
Yes, because he was exerting himself.
What chest pain is exertions? What changes is seen?
Myocardial infarction and the pain causes ST elevation changes on ECG
Second: Ask the patient if the chest pain changes with respiration? What would be the diagnosis?
MI doesn’t change with respiration, pleuritic pain changes so think of pericarditis, pneumonia, pE
Ask the patient if the chest pain is tender on Palpation?
If the pain is tender on palpation, Think of Costochondritis
What are the differences between angina pectoris and myocardial infarction?
Angina shows st depression or normal and is a warning
Mi shows st elevation or NSTEMI and it is an emergency, damage may occur, can also be due to blood clots
Treatment of costochondritis
Ibruprofen
What is the goal of treatment of MI?
Prompt diagnosis and early intervention
Relieve of Chest Pain
Stabilize the heart rhythm
Reduce cardia workload
Revascularization
Name some er management of MI:
Oxygen, morphine for pain, nitroglycerin, aspirin to chew on, loading dose of clopido,maximum dose of statins, ace inhibitors to reduce preload and after load, beta blockers to decrease cardiac load, anticoagulants and thrombolytics

Nitrogen,
Antiplatelet therapy after acute coronary syndrome Dual Antiplatelet therapy DAPT is what?
Indication: Drug eluted stent
Antiplatelet: Clopidogrel and Aspirin
Duration: 12 months and re-evaluate
