Comes in regular or liposomal
Big note: CI in cardiopulmonary arrest
Can Cause: fever, chills, headache, malaise, rigors hypokalemia, hypomagnesimia, and nephrotoxic
ONLY COMPATIABLE WITH D5W
Flucytosine
Myelosupression
Azole Antifungals
Fluconazole, Voriconazole, Posaconazole
Posaconazole needs to be taken with food
Fluconazole
Vagininal Candidiasis 150 mg po once
IV ratio 1:1
Remember increases LFTS and QT prolongation. Renally cleared so dose adjust except for Keto
Echinocandins
Capsofungin and Micafungin Dosing for Micafungin Candidemia: 100 mg IV d Esophageal Candidiasis: 150 mg IV d All cause Histamine mediated sxs
Nystatin
Oral suspension needs to be shaken well
Swish in the mouth for as much as possible then swallow
Terbinafine
Oral terbinafine are hepatotoxic
Can take several months to see results.
Candida Albicans Treatment
Mild: Topical antifungals
Moderate to severe or HIV+: Fluconazole
Candida Albicans Esophogeal infection
Fluconazole
Candida Krusei
Echinocandin or AMPB
Aspergillus
Voriconazole or AMPB,
Cryptococcus Neoformans
AMPB+Flucytosine or High dose Fluconazole+Flucytosine
Dermatophytes
Terbinafine, itraconazole or Fluconazole
