Drugs and the Thyroid Flashcards
What is T4 and its role
Thyroxine
Storage form
Stored in colloid - lipid based fats
What receptor is used for the thyroid
Through the thyroid nuclear receptor TR
What is the MoA of the thyroid hormone
T3-TR bound stimulates nuclear transcription of target genes, increasing mRNA and protein synthesis
What happens when the thyroid hormone increases
The cellular activity increases
What treatment is used for an increase in the thyroid hormone
thioureylenes
Examples of thiourylenes
Carbimazole
Propylthiouracil
What is significant about carbimazole
18 months maintence
and used with levothyroxine
How do thiourylenes work
Inhibits the iodination of Tyr residues in the thyroid gland
How are thiourylenes taken
Orally
gradual reduction in signs and symptoms
NOT SUITABLE FOR PREGNANCY
AE of thyroid treatment
rash/nausea
rare SE - granulocytosis
foetal T4 can be reduced in pregnant
What does propanol do
Inhibits T4 to T3
conversion in the peripheral tissues
What does radioactive iodine do and when is it used
damages thyroid follicle cells
Used in patients who relapse
What is used for hypothyroid treatment
Levothyroxine
What are drugs that affect thyroid hormone synthesis
Thioureylenes
Iodides
lithium
What effect does lithium have
Decrease in thyroid function
