What are the indications for hydrocortisone
Severe life threatening asthma
Acute exacerbation of COPD
Adrenal crisis (Addisons)
Prevention of adrenal crisis in a patient who has suffered traumatic injury and is also on a programme of long term steroid therapy
Pregnant women with known diagnosis of Addisons who are in active labour.
What is the repeat dose for hydrocortisone?
None
Hydrocortisone is a single 100mg dose
What drug can we give for Cocaine toxicity and what are the indications?
Diazepam
Known cocaine use with chest pain, severe Hypertension (>220 systolic >140 diastolic) or convulsions
Adult dose of salbutamol (neb)
5mg in 2.5ml
Repeat every 5 minutes as required
Adult dose Ipratropium Bromide (neb)
500mcg in 2ml (no repeat dose)

What is the dose for GTN for a suspected ACS patient?
400-800micrograms (mcg) every 5-10 minutes with NO maximum dose.

What is the Adult dose of Aspirin for a suspected ACS patient?
300mg
Can you administer aspirin to a Patient who is <16 YO?
No as there is a risk of Reyes disease.
When should adrenaline 1 in 10,000 be given in cardiac arrest? (Adult)
Non shockable rhythm- administer 1mg (10ml) as soon as IV access achieved
Shockable rhythm - after the 3rd and 5th shock followed by every alternative shock.
Diazepam indications
Active convulsions, seizures lasting >5minutes, Eclamptic pregnant mothers with seizures lasting >2-3 minutes. Cocaine toxicity (with chest pain and severe hypertension/ seizures)

Diazepam Actions
Diazepam binds to the benzodiazepine receptors on the chloride channels in the membrane of neurones causing the channel to open allowing chloride ions to enter (Cl-). The refractory period of the neurone is increased) causes the neurone to be unable to reach threshold potential inhibiting signals.
What is Atropine Sulphate used to treat?
Absolute bradycardia and AV block
Actions of Atropine Sulphate
Atropine is an acetylcholine antagonist that prevents action potentials crossing synaptic clefts in the vagus nerve reversing vagal overdrive, allowing the SA node to produce an intrinsic depolarising rate. Also increases conductivity of the AV node in AV block.
