Heparin

A

Given IV or sub Q

Heparin uses

A

Converting from heparin to warfarin

Warfarin Coumadin monitoring

A

INR to be between two and three

INR daily until therapeutic range for two consecutive days
Then two or three times weekly for one or two weeks

Guidelines for reversing warfarin

A
