N/V, increased liver enzymes, increased PT at 24-48 hrs
Acetaminophen OD
Dx blood level
Tx: charcoal if within 4 hours of ingestion
N-acetylcysteine within 8 hours
mucosal burn, dysphagia and drooling
alkali agents ex batteries
NPO
delirium, mydriasis, fever, urinary retention
Anticholinergic
Tx: Physostigmine
white horizontal stripes on fingernails, N/V/D, seizures and shock
Arsenic
Tx: Dimercaprol
Tinnitus, respiratory alkalosis, anion gap metabolic acidosis, nml serum osm
ASA
Tx: bicarb, dialysis
Slurred speech, ataxi, drowsiness
Benzodiazepines
Tx: flumazenil if iatrogenic OD
Bradycardia, heart block, obtundation, hyperkalemia
B-Blocker
Glucagon and IV calcium
Headache, N/V, confusion, coma, cherry-red skin
CO
dx: carboxy-hemoglobin
Tx: 100% O2 or hyperbaric oxygen
almond-scented breath, coma
Cyanide
Antidote: sodium nitrate, sodium thiosulfate, hydroxocobalamine
yellow halos, bradycardia, heart block, hyperkalemia
Digoxin
Dx: antidigoxin, Fab fragments
Calcium oxalate crystals in urine, anion gap, metabolic acidosis with high sOsm
Ethylene glycol
Dx: Osm gap
Tx: Fomepizole
Bleeding and thrombocytopenia
Heparin
Dx: increased PTT
Tx: protamine
Vomiting, bloody diarrhea, acidosis, CXR showing radiopaque tablets
Iron
Tx: Deferoxamine
Confusion, peripheral neuropathy (seizure)
Isoniazid
Tx: Pyridoxine
Microcytic anemia with basophilic stippling, ataxia, retardation, peripheral neuropathy
Lead
Tx: EDTA, Dimercaprol
