Lispro
Quick, short acting insulin
Only give if pt about to eat
Regular insulin
Short acting insulin
Gold standard for insulin
Usually given with NPH to mimic natural insulin levels
NPH insulin
Intermediate release insulin
Acts as basal rate
Usually given with Regular Insulin
Glargine
Long acting
No peak
Acts as very little amount secreted by pancreas thru the day
Metformin
Diabetic adjunctive agent
Drug of choice for DM2
MOA: Inhibits liver’s glucose production, Insulin sensitizer
AEs: GI distress and lacticacidosis
DOES NOT INDUCE HYPOGLYCEMIA –> does not increase insulin in the blood
Glipizide
Diabetic adjunctive agent
MOA: Requires working pancreas –> stimulates pancreas to release insulin
AEs: Hypoglycemia, do not give in preg
Repaglinide
Diabetic adjunctive agent
MOA: stimulates pancreas to release insulin like Glipizide. If one doesn’t work, the other won’t either
AEs: RAPID hypoglycemia, rapid half life
Sitagliptin
Diabetic adjunctive agent
MOA: Stimulates insulin production and suppresses glucagon release by inhibiting enzyme DPP-4
AEs: URI, H/A, inflammation of nasal passages and throat, hypoglycemia, pancreatitis
Liraglutide
Diabetic adjunctive agent
MOA: Incretin mimetic –> Stimulates insulin production and suppresses glucagon release
AEs: GI distress, pancreatitis
- *Decreasse glipizide (sulfonyurea) dosing if used in conjunction with liraglutide**
- *Adminstered SQ!**
Pioglitazone
Diabetic adjunctive agent
MOA: Inhibits liver’s glucose production, Insulin sensitizer
AEs: hypoglycemia, fluid retention, sinusitis and RTIs, CYP450
- *cannot be used as a solo agent**
- *Do not give to heart patients!**
