Albuterol
Ipratropium Bromide
Albuterol:
- Adult = 5mg
- Ped = 2.5mg
Ipratropium Bromide:
- Adult = 0.5mg
- Ped = 0.25mg (if below 15kg)
Aspirin
324-325mg (comes in 81mg pills normally)
Calcium (look at S&Ss of hyperkalemia)
Adult & Ped = 10mg/kg
D50
Adult = 1mg/kg up to 50ml Ped = must use D25 and give 2mg/kg up to 50ml (if under 25kg)
(There’s 25g in a bag of 50ml)

Versed (Midazolam)
Adult & Ped = 0.1mg/kg to max of 5
Diltiazem
- Adult = 0.25mg/kg slow IVP
Indication = Tachycardia - Stable
(Sustained narrow-complex tachycardia > 150 bpm in adults WITH systolic BP ≥ 100 mmHg)

Diphenhydramine
- Adult = 50mg
- Ped = 1mg/kg
Dopamine
- Adult = 5-20mcg/kg/min
Colorado down and dirty = (pt lbs/10) - 2 = 5mcg/min with 10 gtts
how to calculate and admin
Etomidate
- Adult = 0.3mg/kg over 15-30sec
Fentanyl
- Adult = 1mcg/kg up to 100mg (IV, IN, IM) (Can repeat every 10 min up to 3mcg or 250mg max)
- Ped = OLMC ONLY but typically 1mcg/kg up to 50mg max
Morphine
- Adult = 2-4mg can repeat every 5 to max of 10mg
- Ped = OLMC ONLY 0.1mg/kg to max of 2mg per dose
Glucagon
- Adult = 1mg IM
- Ped = 0.5mg IM
Oral Glucose
- Adult = 15g (1 tube)
- Ped = 7.5g (1/2 tube)
Haloperidol
Adult = 5mg IM
Hydroxocobalamin (Cyanokit for Cyanide Poisoning)
- Adult = 5g
- Ped = 70mg/kg
(both done over 15 minutes)
Magnesium Sulfate
1g
Solu-Medrol
- Adult = 125mg
- Ped = 2mg/kg (max of 125mg)
Naloxone
- Adult = 0.5mg, may repeat to max of 4mg (can do 2mg if severe)
- Ped = 0.5mg to max of 2mg
Nitro
- Adult = 0.4mg every 5 minutes so long as BP is above 100 and chest pain persists
Sodium Bicarb
Adult & Ped = 1mEq/kg to max of 50mEq
Epinephrine (asthma/dyspnea; anaphylaxis)
DYSPNEA - ASTHMA (Severe & Refractory to Nebulization)
- Adult = 1:1000 0.3 mg IM
- Pediatric = 1:1000 0.01 mg/kg (0.01 mL/kg) not to exceed 0.3 mg (0.3 mL) IM
ANAPHYLAXIS
- Adult = 1:1000 0.5 mg IM
AND IF anaphylaxis refractory to above IM dose:
1:10,000 1 mg slow IVP/IOP over 3 minutes
- Pediatric
1:1000 0.01 mg/kg (0.01 mL/kg) not to exceed 0.3 mg (0.3 mL) IM
AND IF anaphylaxis refractory to above IM dose:
1:10,000 0.01 mg/kg slow IVP/IOP over 3 minutes
Rocuronium
- Adult = 1mg/kg
- Ped = 0.6mg/kg
Pediatric Arrest Cardioversion, Epi and Amio
Cardioversion = First shock - 2J/kg ; Second shock 4J/kg ; all after shocks - 4-10J/kg Epi = 0.01mg/kg max of 1mg every 3-5 Amiodarone = 5mg/kg repeat up to 3 times
Pediatric Brady Epi and Atropine
Epi = 0.01mg/kg up to 1mg
Atropine = 0.02mg/kg (min is 0.1mg and max is 0.5mg) may repeat 1 time
Pediatric Tachycardia Sync Cardiovert and Adenosine
Sync Cardiovert = 1J/kg and if not effective move up to 2J/kg
Adenosine = First dose - 0.1mg/kg ; Second dose - 0.2mg/kg (max of both is what the adult dose is (6&12))

Cuffed = (age in years/4) + 3.5
