Error, no harm; an error occurred that resulted in the need for increases patient monitory but caused no harm
Category D
Error, harm; an error occurred that resulted in the need for treatment or intervention and causes temporary harm
Category E
Error, harm; An error occurred that resulted in initial or prolonged hospitalization and caused temporary harm
Category F
Error, harm; an error occurred that resulted in permanent patient harm
Category G
Error, harm; an error occurred that resulted in a near death event (anaphylaxis, cardiac arrest)

An error occurred resulting in death

Circumstances that have the capacity to cause error (no error)
