What poses heightened toxicity risk/interacts with digoxin?
CIx in:
Hypomagnesmia
Hypokalemia
Hypercalcemia
Quinidine, Verapamil, and Amiodarone DECREASE digoxin clearance (increase effects/AE)
CIx in a-fib, a-flutter with accessory pathways (ex: WPW)
B blockers
CIx in:
Bradycardia
Severe asthma or bronchospasm (or according to review, maybe not?
Depression (propranolol specifically)
Severe hypotension/cardiogenic shock
Pindolol and Acebutol
CIx post-MI or HF
nDHP CCB
CIx in: Severe hypotension Cardiogenic shock LV dysfunction (EF <40%) Bradycardia Conduction defects Systolic HF (HFrEF)
**CYP3A4 inhibitors!
Don’t use with B blockers, simvastatin, lovestatin, digoxin (Verapamil)
Class IC anti-arrhythmics: Flecainide, Propafenone
CIx in patients with CAD
Sotalol (Class II anti-arrhythmic)
CIx in patients with:
LV dysfunction
HR <60
Dofetilide
CIx in patients taking Verapamil, hydrochlorothiazide
Amiodarone
CYP3Y4 and CYP2C9 inhibitor: warfarin, digoixn, statin interactions
CIx in pts with thyroid dysfunction
DC immediately if pulmonary fibrosis occurs
Dronedarone
CIx in patients with:
Severe HF
Hepatic dysfunction
NTG
CIx in patients with right ventricular infarction
Sodium Nitroprusside
Caution in patients with CAD
Intracranial lesion/encephalopathy/recent brain surgery (due to increase in intracranial pressure)
Hydralazine
CIx in patients with CAD
