Rejected Takeoff Drill - CAPTAIN
“CONTINUE / REJECT”
if REJECT:
- Retard Thrust Levers to Close
- Disengage Autothrottle
- Observe or Apply Maximum Braking
- Raise the Speedbrake Lever UP
- Select Maximum Reverse thrust consistent with conditions
(announce “MANUAL BRAKING” if reject below 90 kts)
Rejected Takeoff - FIRST OFFICER
- “SPEEDBRAKES UP” or “NO SPEEDBRAKES”
- “REVERSE GREEN” or “NO REVERSE NUMBER 1/2”
- “60” and advise ATC
Ground Incident Procedures (Generally Captain Duty)
If immediate evac NOT required
- “REMAIN SEATED, REMAIN SEATED”
If urgent contact with IC req’d, PA
- “IN-CHARGE FA CALL (OR REPORT TO) THE FLIGHT DECK”
If evac req’d
- “CHECKLIST, PASSENGER EVACUATION”
Driftdown Procedure - PF
“DISENGAGE AUTOTHROTTLE”
“SET MAX CONT THRUST”
- Trim as required
Confirm thrust mode changes to CON and thrust is MCT
- Request ENG OUT to be selected on ECON CRZ page
Allow speed to slow to target a/s
LVL CHG…………… Engage
- Initiate Turn if required
“CHECKLIST, ENGINE FAIL”
Driftdown Procedure - PM
On Request..
- Disengage Autothrottle, Select CON thrust in FMC, Set MCT
- ENG OUT prompt…… Select
- FAILED ENG prompt…… Select
Determine driftdown requirement
- Set MAX ENGINE OUT ALTITUDE on MCP
- Set ENG OUT TARGET AIRSPEED on MCP
- ALL EXT LIGHTS…………… ON
- ATC……………………………. NOTIFY
Read C/L on request
Rapid Decompression- CAPTAIN
“CAPTAIN ON OXYGEN”
- Seatbelt Sign…… ON
“ATTENTION FLIGHT ATTENDANTS SECURE THE CABIN, PASSENGERS TAKE YOUR SEATS”
If Cabin uncontrollable:
- Set lowest safe alt or 10,000’
- Initiate turn if required
- LVL CHG
- Ensure Thrust Levers closed
- Extend Speedbrakes
- Set VMO/MMO
“CHECKLIST, CABIN ALTITUDE WARNING OR RAPID DEPRESSURIZATION”
- When 2000’ above, reduce speed
- When 1000’ above, retract speedbrakes
Rapid Decompression - FO
“FIRST OFFICER ON OXYGEN”
“CABIN OK” or “CABIN UNCONTROLLABLE”
- Pass Oxygen……… ON
- Seatbelts …………Ensure ON
- Landing Lights ………….. ON
- ATC………………….ADVISE
- Squawk…………….. 7700
PA “ATTENTION, PUT ON YOUR OXYGEN MASKS IMMEDIATELY”
Read checklist silently
Once Level… PA “IN CHARGE F/A CALL (OR REPORT TO) THE FLIGHT DECK” using o2 if req’d
TCAS RA (clean and landing config)
PF
If maneuvering is required:
- Disengage autopilot and authothrottle
- Smoothly adjust pitch and thrust to satisfy RA command
in Landing Config:
- Disengage autopilot and autothrottle
- Advance thrust levers forward to max thrust
- Call for FLAPS 15 and smoothly adjust pitch to satisfy RA command
- Continue lateral path unless visual manoeuvring with traffic is required
- “POSITIVE RATE” / “GEAR UP”
Notify ATC - “AC___, TCAS RA”
(TR 24)
Upset Recovery - Nose Down
Recognize and Confirm Situation
PF
- Disengage Autopilot and Autothrottle
- Recover from stall, if required
- Roll in shortest direction to wings level
- Recover to level flight
PM
- Call out Attitude and Airspeed throughout recovery
- Verify required actions are completed
GPWS (“PULL UP”)
PF
- “FIREWALL”
- Disengage Autopilot and Autothrottle
- Apply Max Thrust Aggresively
- Roll Wings Level and pitch to 20*
- Retract Speedbrakes
Do not change config and monitor terrain separation
PM
Assure Max thrust
Monitor VS and ALT
- Call any trend towards terrain
Approach to STALL or STALL
“STALL”
- Hold Control Wheel
- Disengage AP and AT
- Smoothly apply nose down to reduce AoA
- Wings level if needed
- Thrust as needed
- Retract Speedbrakes
If during liftoff with flaps up
- “Flaps 1”
Windshear Escape Maneuver
“WINDSHEAR TOGA”
If Manual Flying:
- Disengage AP
- TOGA and apply max thrust
- Disengage AT
- Roll Wings Level and pitch 15°
- Retract Speedbrakes
- Follow FD, if available
If AP engaged:
- TOGA and verify
- Verify GA Power
- Retract Speedbrakes
- Monitor
Do not change config until clear of alert
(TR 24)
Low Energy Go-Around
- Contact with the runway may occur
- Speed of Vref should be achieved prior to commencing climb
Normal Go-Around Procedures….
Go Around - One Engine Inoperative
If a flap 15 approach is used……… Use flap 1 for go around
Limit Bank angle to 15 degrees until out of orange band (lower of Vref 15+15 or Vref ICE +5 is attained)
If flap 30 approach….. use flap 15 for go around
If LAND 3 or LAND 2 is annunciated:
Be aware of rudder input at 400 AGL when AP reverts to Single AP Operation
Engine Failure During Go-Around / Missed Approach
- Perform Normal Go Around
- Maintain Flaps 15, Vref 30 or 40 plus wind (command speed) and limit bank to 15° until safe altitude is reached
- Accelerate then retract flaps on schedule
