cefepime
4th gen cephalosporin. covers gram positives AND pseudomonas. stable against most beta lactamases. No anaerobic coverage
ceftaroline
3rd/5th gen cephalosporin. binds to different PBPs, so can cover MRSA. need an ID consult to give.
carbapenems: general coverage, principles
Great CNS penetration. cover many gram negatives. closely related to penicillins with many cross allergies. higher than average seizure risk.
Antibiotics that treat pseudomonas
Zosyn (pip+tac). First line.
also imipenem, meropenem, azotreonam, ceftrazidme (3rd gen), cefepime (4th gen), quinolones (-floxacin), aminoglycocides (gentimicin etc)
Antibiotics that treat anaerobes
Metronidazole (flagyl): first line
also clindamycin, Zosyn (pip-taz), unasyn (amp + sulfabactam), imipenem, meropenem, cefotetan (2nd gen), tigecycline, levofloxacin and moxiflxacin
Drugs that treat atypicals
tetracyclines, macrolides, aminoglycosides, ciprofloxacin (and other quinolones?)
Drugs with CNS penetration
imipenem, meropenem, cefepime (4th gen), vancomycin (ok), ceftriaxone, cefotaxime (both 3rd gen), ceftazidme (3rd gen), linezolid
Drugs for MRSA
vancomycin, ceftaroline, linezolid, tigecycline, clindmycin, daptomycin
Drugs for VRE
linezolid, tigecycline, daptomycin
general empiric broad spectrum regimen
vancomycin for gram positives, enterococci, MRSA, and C diff, and pip-taz (zosyn) for broad gram neg, pos, pseudomans, and aerobic coverage.
