What must the collector do if there is a sink with running water in the toilet area of the collection site?
The collector must ensure that the sink is secured in a way that will not allow the donor to access the water.
This may be accomplished by shutting off the water supply and using tamper evident tape to secure the faucet.
What three things must your collection site have procedures for?
- Ensuring that only authorized personnel are present and that access to the collection site and collection materials are restricted.
- Ensuring that authorized people are supervised while at the collection site.
- Secure handling and storage of specimens.
What is the one exception for the one collection at a time rule?
You may conduct a collection on another donor if you are in the middle of a shy bladder-process with a separate donor (while the shy bladder donor is waiting and being washed by someone else).
Collection kit
You must use a single use collection cup, made of plastic, that will hold at least 55 ml of urine.
The cup must have clear markings at the 45 ml line and above.
The cup must have an external temperature strip that shows readings of 32 to 38° C / 90 to 100° F.
The specimen bottles
You will need two specimen bottles (these are used to ship the specimens to the lab).
The bottles must have markings indicating 30 ml for the primary bottle and 15 mL for the split.
The bottles must have lids that prevent seepage of urine from the bottles.
The bottles must have some tamper evidence seal on them.
How should the specimen bottles and paperwork be shipped to the lab?
The labs copy of the paperwork (CCF - Custody and Control Form) must be sent to the lab in a leak resistant plastic bag that has two pouches.
In the pouch for the urine specimen, there must be an absorbent pad.
The bag needs to be tamper resistant once the contents are sealed inside.
What is the tamper evident tape used for?
To secure faucets, toilet tank lids, etc. from being tampered with.
Can you do the collection without proper identification?
No. You must get proper ID from the donor before beginning the collection.
What are the acceptable forms of identification for. Testing?
A government issued photo ID such as a driver’s license, passport or picture ID issued by federal, state or local government agency.
Employer issued photo IDs such as an employee badge with the employees picture.
Other identification allowed by the operating agency rules that cover the employee.
Can you accept an expired ID from the donor?
Yes. You may accept an approved form of identification that is expired so long as it’s not expired for more than one year.
Unacceptable. What forms of ID are not acceptable for. Testing?
Identification by a co-worker or another safety sensitive employee.
Non-photo ID such as a credit card, library card, social security card, etc.
Fax or photocopies of the donor’s ID.
What do you do if the donor does not have proper photo ID?
The collector should contact the der at the company and ask the der to positively identify the employee. Then the collector would make a note in the remarks section of the CCF (Custody and Control Form).
EX. “ DONOR DID NOT HAVE PROPER PHOTO ID. DER, LINDA SMITH, IDENTIFIED EMPLOYEE.”
What is the one exception to the ID rules?
If the person is self-employed such as an owner-operator, truck driver. This particular DOT covered worker does not have a DER that you can call to identify him. Therefore, you are allowed to use what is called “signature identification” for this person if he or she comes into your collection site without proper photo ID.
If a Self-Employed individual does not have proper identification, how is this situation handled?
In this situation, you use “signature identification”.
For “signature identification”:
In addition to the driver’s CDL number which is used as the ID number donor must provide two pieces of non-photo ID that the donor has with him or her.
These pieces of identification must already contain the donor’s signature for comparison purposes.
Proceed with the collection, and when the donor completes Step 5 Copy 2 (certification statement), compare the signatures. If the signatures match make a note in the remarks.
EX. DONOR DID NOT HAVE PHOTO ID. SIGNATURE ID CONFIRMED (SS CARD AND CREDIT CARD).
