Dosages Flashcards
Adenosine
Adult: 12mg rapid IV bolus followed by flush. Repeat once followed by flush if needed.
Pediatric: Base contact, 0.1mg/kg IV bolus max 6mg rapid followed by flush. 0.2mg/kg IV bolus max 12mg followed by flush if needed.
Amiodarone
Adult Pulseless Arrest: 300mg IV bolus additional 150mg IV bolus in 3-5 minutes if refractory or recurrent.
Adult Symptomatic with pulse: Base contact for 150mg IV bolus over 10 minutes.
Pediatric Pulseless Arrest: 5mg/kg IV bolus. Contact base for additional doses.
Aspirin
324mg PO
Atropine
Adult: 0.5mg IV/IO bolus repeat if needed 3-5 minutes to a maximum dose of 3mg.
Pediatric: 0.02mg/kg IV/IO bolus minimum 0.1mg maximum single dose 0.5mg.
Calcium
Adult: Gluconate(Chloride): Arrest due to hyperkalemia- 3g(1g) slow IV push
Adult: Gluconate(Chloride): Calcium channel blocker- Base contact 3g(1g) slow IV push every 10 minutes for max of 3 doses.
Pediatric: Gluconate(Chloride): Calcium Channel Blocker: Base Contact 60mg/kg max of 1g (20mg/kg) IV/IO push repeat 10 minutes for 3 total doses.
Epinephrine
Adult Arrest: 1mg 1:10 IV/IO every 3-5 minutes
Pediatric Arrest: 0.1ml/kg 1:10 IV/IO every 3-5 minutes
Adult Bradycardia: Dirty Epi Drip
Pediatric Bradycardia: Base contact for 0.1mL/kg 1:10 IV/IO
Adult Anaphylaxis/Asthma: 0.3mg 1:1 IM repeat x1
Pediatric Anaphylaxis/Asthma: 0.01mL/kg 1:1 IM or 0.15mL 1:1 IM <25kg and 0.3mL 1:1 IM for >25kg
Magnesium Sulfate
TdP: 2gm IV bolus
Bronchospasm: 2gm IV Bolus over 2 minutes
Eclampsia: 2gm IV slowly, 4gm/50mL NS over 15-30 minutes
Nitroglycerin
Chest pain: 0.4mg Sublingually every 5 minutes x3 doses
Pulmonary Edema: 0.4mg Sublingually every 5 minutes PRN
Sodium Bicarbonate
Adults/children >10kg, 8.4% Tricyclic OD or hyperkalemia:
1mEq/kg slow IV push repeat every 10 minutes as needed
