A

Adult: 12mg rapid IV bolus followed by flush. Repeat once followed by flush if needed.
Pediatric: Base contact, 0.1mg/kg IV bolus max 6mg rapid followed by flush. 0.2mg/kg IV bolus max 12mg followed by flush if needed.

A

Adult Pulseless Arrest: 300mg IV bolus additional 150mg IV bolus in 3-5 minutes if refractory or recurrent.
Adult Symptomatic with pulse: Base contact for 150mg IV bolus over 10 minutes.
Pediatric Pulseless Arrest: 5mg/kg IV bolus. Contact base for additional doses.

A

Adult: Gluconate(Chloride): Arrest due to hyperkalemia- 3g(1g) slow IV push
Adult: Gluconate(Chloride): Calcium channel blocker- Base contact 3g(1g) slow IV push every 10 minutes for max of 3 doses.
Pediatric: Gluconate(Chloride): Calcium Channel Blocker: Base Contact 60mg/kg max of 1g (20mg/kg) IV/IO push repeat 10 minutes for 3 total doses.
