Albuterol
- Adult: 2.5-5mg diluted to 3mL with NS; admin over 5-15 min; 3-4 times per day
*Note: In severe asthma or hyperkalemia doses up to 10-20mg may be necessary - Ped: 0.1-0.2 mg/kg per dose diluted in 2mL NS; <44lbs (20 kg): 2.5mg per dose; >44lbs: 5mg per dose
Dexamethasone
- Adult: 4-20mg/day in single dose or divided; high dose: 0.4-0.8mg/kg (max 40mg/day)
- Ped: 1 dose of 0.6mg/kg (max 10mg)
Diphenhydramine
- Adult: 1mg/kg (max 50mg), IM/PO or slow IV push
- Ped: 1mg/kg (max 25mg)
Racemic Epinephrine
- Adult: Dilute 0.5mL (1% or 2.25%) in 5mL saline over 15 min
- Ped: 2.25% inhalation solution (0.5mL of 2.25% solution mixed in 2.5mL NS; may rpt in 20 min
Epinephrine (Transient hypotension)
Adult: Push dose (10mcg/mL). 0.01mg/kg to max dose of 10mcg. May redose q 5-10 min.
Ped: N/A
Epinephrine (Pulseless arrest)
Adult:
- IV / IO: 1 mg (10mL of 0.1mg/mL solution) push or ET tube (2-2.5 mg diluted in 10mL of NS), rpt q 3-5 min. (follow each IV dose with a 20mL saline flush). Elevate arm for 20-30 sec after dose. Higher doses (up to 0.2mg/kg) may be used for specific indications (ie beta blocker)
Ped:
- IV/IO: 0.01mg/kg (0.1mL/kg of 0.1mg/mL solution) q 3-5 min, max dose 1 mg
- ET tube: 0.1mg/kg of 1mg/mL solution q 5 min until IV/IO access
Epinephrine (anaphylaxis/bronchoconstriction)
Adult:
- IM: 0.3mg (0.3mL of 1mg/mL solution) IM in anterolateral thigh; rpt in 5-15 prn
- IV infusion: 0.02-0.3mcg/kg/min if shock persists after rpt IM doses
Ped:
- IM: 0.01 mL/kg (0.1mL/kg of 1mg/mL solution) IM, max single dose of 0.3 mg; rpt in 15 min prn
Epinephrine (Croup)
3mg (3mL of 1mg/mL solution mixed with 3mL NS) nebulized
Etomidate
Adult: 0.2-0.6 mg/kg (often 0.3mg/kg) IV over 30-60 sec; limit to 1 dose
Ped (>10 years): 0.2-0.4 mg/kg for sedation infused over 30-60 sec; max dose 20mg
Ipratropium Bromide
Adult: 0.5mg nebulized; rpt twice
Ped: 250-500mcg (nebulized or MDI) q 20 min for 3 doses
Ketamine
Delirium:
Adult: 2mg/kg IV over 1 min or 4mg/kg IM
Child (>2years): 1mg/kg IV over 1 min or 3mg/kg IM
Pain mgmt:
0.25 mg/kg IM/IV/IO (max initial dose 25mg; max cumulative dose 100mg)
Magnesium Sulfate (status asthmaticus)
Adult: max dose 2g over 10-15 min
Ped (<12 years): Standard dose: 50mg/kg (0.2mmol/kg) IV, with range 25-75 mg/kg IV, given over 20 min, up to 2g max
Methylpredisolone
Adult: 2mg/kg up to 125mg IV, or 40-125mg IV
Ped: Loading dose: 2mg/kg IV (max 60mg)
Midazolam (anxiety dose)
Adult: 5mg IV/IM/IN
Ped:
- IV: 0.05-0.1mg/kg IV (max dose 5mg)
- IM: 0.1-0.15 mg/kg (max dose 5mg)
- IN: 0.3mg/kg IN (max dose 5mg)
Midazolam (Seizures)
- Adult:
-IN/IM: 0.2mg/kg (max dose 10mg) IN or IM (preferred) - IV: 0.1 mg/kg up to 4mg (consider starting with 1-2.5mg slow IV push in adults)
-Ped: 0.05-0.1 mg/kg per dose
Morphine (Pain)
Adult/Ped: 0.1mg/kg IM/IV/IO (max initial dose 10mg)
Morphine (End of life dyspnea)
Adult: 1-5mg IV/IM/subQ, rpt prn
Nitroglycerin (acute pulmonary edema)
- Adult:
- In severe distress, consider 0.8mg (2 tabs) if SBP>160 or 1.2mg (3 tabs) if SBP>200, q 5 min.
- If not in severe distress, begin at rate of 10mcg/min; increase by 10mcg/min q 3-5 min; ceiling dose of 200mcg/min
- Ped: continuous infusion: initial dose 0.25-0.5mcg/kg/min; titrate by 1mcg/kg at lease q 15-20 min; max dose 20,mcg/kg/min
Nitroglycerin (Chest pain)
- Adult: 0.4mg sublingual, may rpt for total of 3 doses at 3-5 min interval
Phenylephrine (none)
N/A
Succinylcholine
- Adult/Ped: 1-1.5mg/kg IV/IO; 3-4mg/kg IM
- Infants: 2mg/kg IV/IO
Acetaminophen PO
- Adult: 325-1,000mg q 4-6 hours prn; max daily dose of 4,000 mg/day
- Ped: 10-15mg/kg q 4-6 hours; same as adult for children >= 12 y/o
Vasopressin
0.03-0.04 units/min continuous infusion
Lidocaine (Cardiac arrest from VT/VF)
- Adult: 1-1.5mg/kg IV/IO bolus or ET (at 2-2.5 times IV dose); for refractory VF may give additional 0.5-0.75 mg/kg IV push, rpt in 5-10 min; max 3 doses or total 3mg/kg
- Ped: 1mg/kg IV/IO loading dose
Atropine (bradycardia)
- Adult: 1mg q 3-5 minutes for desired response (max total dose: 0.04mg/kg [3mg]); ET dose: 1-2mg
- Ped: 0.02mg/kg IV/IO; max total dose 1mg
Atropine (Anticholinesterase)
- Adult: 1-2mg IV push q 5-15 min until effects observed, then q 1-4 hours for at least 24 hours
- Ped (<12 years): 0.05 mg/kg/dose IV/IO, may be repeated, doubling dose q 5 min
- Ped (>=12 years): 1mg IV/IO, doubling dose q 5 min
Naloxone
Adult: 0.4-2mg IV/IM up to 4mg IN or 5mg ET; can administer up to 6-10mg under 10 min if suspect opioid overdose
Ped: 0.1mg/kg IV/EM/ET (max 2mg) or IN (max 4mg)
Adenosine
Adult:
- Initial dose: 6mg rapid IV or IO bolus over 1-3 sec, followed by 20mL saline bolus, then elevate extremity
- Second dose (12mg) may be given in 1-2 min if needed
Ped:
- Initial dose 0.1mg/kg rapid IV or IO (max dose 6mg)
- Second dose 0.2mg/kg (max 12mg)
Amiodarone (Pulseless VF or VT)
- unresponsive to CPR, shocks, and epi
Adult: 300mg IV/IO push; if needed second dose of 150mg
Ped: 5mg/kg rapid IV/IO bolus, can be repeated to total dose of 15mg/kg per 24 hrs; max single dose 300mg
Amiodarone (life-threatening dysrhythmias)
Adult: Max cumulative dose 2.2g IV per 24 hrs; rapid infusion 150mg over 10 min. rpt q 10 min. if VT recurs
Ped: loading dose 5mg/kg IV/IO over 20-60 min (max single dose 300mg)
Sodium bicarbonate
Adult: 1mEq/kg (50-100 mEq) IV
Ped: 1mEq/kg IV; infuse slowly and only if ventilations are adequate
Aspirin (ACS)
Adult: 160-325mg
Fentanyl
Adult: 1mcg/kg IV/IO (max initial dose 100mcg)
Ped: 1mcg/kg
Calcium chloride
Adult: 500-1000mg (5-10mL of 10% solution), do not exceed 1mL/min; repeat prn
Ped: 20mg/kg (0.2mL/kg) of 10% solution slow IV/IO; dose should not exceed adult dose
Dopamine
Adult: usual infusion rate 5-20 mcg/kg/miin; titrate to response; taper slowly
Verapamil
Adult:
Initial– 2.5-5mg slow IV bolus over 2 min
Repeat– 5-10mg bolus in 15-30 min after initial
Ped:
1-15 years old: 0.1mg/kg (max 5mg) over at least 2 minutes with continuous monitoring
Diltiazem
Adult only:
0.25 mg/kg (15-20mg for avg pt) IV over 2 min; may be repeated in 15 min (0.35mg/kg or 20-25mg for avg pt)
