Dosage Verapamil

A

Initial 2.5-5mg IV/IO over 2 mins

Repeat 5-10mg in 15-30 mins. Max 20 mg

A

Dosage Lidocaine

A

Adult VT w/ pulse: 0.5-1mg/kg slow IV/IO every 5-10 mins up to 3mg/kg

Adult VT w/out pulse:
1-1.5mg/kg IV/IO blous followed by repeat doses every 5-10 mins to max 3mg/kg

IO admin for numbing during IO infusion: adult- 20-40 mg slow IO push

ROSC: 1-4mg/min

A

Cardiac arrest
• 1mg IVP every 3-5 mins
Rosc
•0.1-0.5 mcg/kg/min
Symptomatic bradycardia
•2-10 mcg/min, mix 1 epi in 250ml D5W
Croup
• adult - not indicated
Anaphylactic shock/ asthma
•0.3-0.5 mg SQ/IM

A

VF/Pulseless VT
•1st dose 300 mg diluted in D5W for total 20-30 ml IV/IO
•2nd dose 150mg diluted in D5W for total of 20-30 ml in 3-5 minutes

Wide complex tachycardia w/ pulse
•150 mg diluted in D5W for total of 50-100 ml over 10 mins

Maintenance infusion
•1 mg/min (100mg in 100ml D5W )

Bradycardia
•1 mg IV/IO/ET repeated every 3-5 mins to max 3 mg

Cholinergic/organophosphate poisoning
•1-4mg IV/IO/ET or IM every 5-10 mins or until atropinization
