فينا نستخدم هاد الدواء لحاله او مخلط

شو الهدف من هاد الدواء
انه نعالج الاشخاص يلي عندهم مشاكل بالمفاصل وما بيستجيبو للNsaids
Rheumatoid or psoriatic arthritis wh do not respond to NSaids

شو فائدة هاد الدواء
يقلل من ظهور الerosion الجديدة في المفصل
كيف الاستجابة لدواء Methotrxate
Occures sooner than is usual for other slow acting agent
يعني خلال ٣ الى ٦ اسابيع من بدء العلاج
هل يعتبر الMethotrxate دواء immunosuppressant
اه لانه بده يقلل من هجوم الWbc وخلايا الاخرى في حالات الarthritis an immume disease

كم الdose يلي بنحتاجها
اقل بكثير من الDose يلي بنحتاجها في علاج السرطان وبنوخذها مرة واحدة في الاسبوع بالتالي الadverse effect اقل
شو الside eefect للMethotrexate
رح يصير mucosal ulceration and nusea
Cytopeniaيلي هية عبارة عن تقليل عدد الخلايا الدم البيضاء
Liver cirrhosis
Acute pneumonia like syndrome
في دواء ثاني بيعطيوه بعد بيوم من استخدام الmethotrxateعشان يخفف من الA.E
Leucovorin
Leflunomideيعتبر من الادية السيطرة ولا العلاج؟
من ادوية السيطرة contoller
سو مبدا عمل الleflunomide
بعمل على تثبيط عمل الDHODH
Dihydroorotate dehydrogenase
يلي كان مهم في تكوين الpyramidne لانتاج الDna and RNA
ارتباط الleflunomide مع الDHODR
Revirsibly
شو العلاج يلي بيعملو الLeflunomide
Reduce pain and inflammation but aslo slow the progression of structural damage
هل بنستخدمه بشكل مفرد ولا مخلط
بيصير على شكل monotherapy كبديل عن الmethtrexate وكمان بينفع تستخدمه كمان combination therapy
Leflunomide ايش الفارماكو كينتيكس؟
Per oral بيرتبط بالالبومين اكثر من ٩٠ بالية
Half life : 14-15 days
ميزة حلوة فيه انه بسرعه بيتحول الى active metabolite
بالتالي بسرعه بنخاض للbiliary recycyling

Urine and feces
What is the averse eefect of leflunomide
Headache , diarrhea , nusea Loss of weight , allergic reactiom including flu like syndrome Skin rash Alopecia الصلع Hypokalemia
Leflunomideبصير نعطيه للحوامل؟
لاء لانه teratogenic
كمان ممنوع للنساء يلي ممكن تصير حامل
Child bearing potential
Leflunomide لازم نستخدمها بحذر وين؟؟؟؟؟؟
في حالات الliver disease
لانها تعتمد على الbile and renal Excretion

في علاج الملاريا
وكمان في علاج الarthritis يلي ما بسنجيبو للNSaids
او بنستدمهم مع استخدام الNSAIDS يلي لازم وقتها نستخدم lower doses of drug
كمان بتعمل على slow progression of erosive bone lesion and many induce remission
ليش ما بنستخدم الCholoroquinone and hydroxychloroqunine
لانه الها serious adverse effect

تعتبر analog of amino acid cysteine
D-penicillamine
Slow the progress of bone destruction and rheumatoid arthritis
D-penicillamineاذا استخدمناه فترة مطولة
رح يسبب serious side effect منها dermatologic , nephritis and aplastic anemia
شو شرط D-penicillamine في علاج الRA
انه نكون قد استخدمنا الgold salt وما زبط في العلاج لكن قبل استخدام الكورتيزون (corticosteroids)
D-penicillamine لايش كمان يستخدم
As a chelator in the treatment of posoning by heavy metals

كيف بيشتغل الazathioprine
من خلال الmajor merabolite , 6 thioguanine which supresses inosinic acid synthesis , B cell snd T cell function , immunolglobulon production , interleukein 2 secretion
What is the pharmacokinetics of azathioprine ?
Bimodal metabolism
Rapid metabolizer clearing the drug four times faster than tge slow metabolizer
المرضى يلي عندهم TMPT قليل او ما في بيكون عندهم خطر الاصابة بالmyelosuppresion by excess con of parent drug if the dosage not adjusmend
