To take immediately in the consult (e.g. emergencies) tigger, treatment, response, detail, severity, frequency and current incident

Non-urgent bits of in information to acquire in an emergency. Signalment, neutered or entire? ownership history, vaccination and worming, surgery previously, current medication, health of in contact animals, housing, feeding, use, general health
