What is diverticulum?
An outpouching at perforating artery sites
What is diverticulosis?
An asymptomatic outpouch
what is diverticular disease?
A symptomatic outpouch
What is diverticulitis?
Inflammation of outpouch
Infection
What % of diverticula is symptomatic and non symptomatic?
95% asymptomatic
5% diverticular disease
What are the risk factors of DD?
Connective tissue disorders (M & ED), Ageing
(multifactorial)
increased colon pressure, COPD, Chronic cough, age
Pathology of DD causing the outpouches and what does this lead to?
increased colon pressure = Mucosa and submucosa bubble out through weak spots of Colon wall (where BV penetrate muscle layers)
BV Rupture = GI Bleeding
Bacteria + undigested food stuck in outpouch = Infection (diverticulitis)
What are the Triad of symptoms?
- LLQ pain (lower left quadrant)
- Constipation
- Fresh rectal bleeding
Diagnosis of DD
CT abdo/pelvis with contrast - GS
Treatment for Diverticulosis, Diverticular disease, Diverticulitis?
LOSIS = Nothing, wait and watch
LAR DISEASE = Bulk forming laxatives - surgery is GS
LITIS = Abx (Co amoxiclav) + paracetamol. IV Fluid, liquid food, rarely surgery
What is Meckel’s diverticulum?
What is the rule of 2s?
Diagnosis test?
Pediatric diverticulum
failure to obliterate vitelline duct
2y/o, 2 inches long, 2ft from ileocecal valve (umbilical)
Dx = Technitium scan
Complication of DD
SBP, bowel obstruction, fistulae
