Q

What happens in renal artery stenosis?

A

Decreased blood flow to kidneys causes a decrease in the GFR. Decreased GFR stimulates efferent arteriole constriction to increase GFR as a result of activation of the RAAS.

Q

What happens when you give an ACEi in renal artery stenosis?

A

Prevents vasoconstriction of efferent arteriole by angiotensin II, therefore causing a reduction in the GFR which causes renal failure.

Q

What changes in prescribing do you need to make in someone with renal disease?

Q

What are the main issues you need to take into account when prescribing in renal disease?

A

Drugs can reduce kidney function by direct of indirect toxicity If reduced kidney function, can get drug accumulating to toxic levels, even at normal doses.
