minimal change dz: nephrotic or nephritic?
nephrotic
minimal change dz?
more in children (following URI)
Adults: NSAIDS lithium, lymphoma
minimal change dz s/s & tx
PROTEINURIA
full blown nephrotic syndrome
tx: prednisone
IGA nephropathy: nephrotic or nephritic?
nephritic
IGE nephropathy? aka?
common in asians, likely hereditary
Berger dz
IGE nephropathy s/s
gross hematuria (urine red or COCA COLA colored)
no latent period 1-2 DAYS post viral infection
post inf. glomerulonephritis: nephrotic or nephritic?
nephritic
post inf glom?
due to inf w/ GAS
appears after PHARYNGITIS OR IMPETIGO
1-3 WEEKS after infection
but lots of other causes
post inf glom s/s?
can vary!
- asymp glom hematuria
- nephritic syndrome (htn, oliguria, edema, coca cola urine)
good pasture syndrome: nephrotic or nephritic?
nephritic
good pasture syndrome?
20-30’s male, 60-70’s no gender prev.
- combo glomerulonephritis and PULMONARY HEMORRHAGE
- tobacco use predisposes
good pasture s/s?
- may be preceded by URI
- hemoptysis, dyspnea, resp failure
good pasture diagnostic data?
- circulating anti GM antibodies
- iron deficiency anemia
polycystic kidney dz: nephritic or nephrotic?
NEITHER! SUCKER
polycystic kidney dz
- hereditary
- 50% will have ESRD by 60y/o
true or false: nephrotic and nephritic syndromes are both affecting the glomerulus?
TRUE
polycystic kidney dz s/s and tx?
s/s
- abd/back/flank pain
- HTN
- family hx
tx
- dont drink caffeine
- treat HTN AGGRESSIVELY to slow progression
diagnostic data polycystic kidney dz
ultrasound:
- 2+ cysts in each kidney if 30-59y/o
- 4+ cysts if >60y/o
analgesic nephropathy seen in who?
pts who ingest COMBINATIONS OF:
phenacetin, paracetamol, aspirin, NSAIDS
often women >45 w/ chronic pain, headaches, arthritis
1g/day for 3+ years
analgesic nephropathy aka?
chronic interstitial nephritis
s/s analgesic nephropathy?
hx of long term consumption of analgesics
no s/s of urinary tract issues
HTN common
