Who gets acute rehab?
Pt needing intensive medical program and more than 3 hrs/day of PT
what is subacute rehab?
pt needs less than 3 hrs/day of PT. can be in rehab facility or qualified nursing home
what is day rehab?
outpatient - can be full or 1/2 day for 3-5 days/wk
patients are ___ oriented and families are ____ oriented
patients are PERSON oriented and families are PROCESS oriented
what does process oriented mean?
look at amount of time spent by d/c planner on their case, look at # of options considered, look at pt’s involvement in the decision
what is LTAC?
long term acute care
who goes to LTAC?
ICU pts…
ventilator care, complex wounds, chronic complex medical condition
how long is typical LTAC stay?
30 days
can pt’s on TPN go to acute rehab?
no
how long is typical acute rehab stay?
12-14 days
how long is typical sub-acute rehab stay?
20-30 days
who goes to sub-acute rehab?
less intense rehab, tolerate 1 hr PT daily, pts who tire easily
location for sub-acute rehab
SNF or nursing home
medicare pt’s must meet what insurance criteria to qualify for skilled nursing care?
3 overnight stay

PT/OT wound care trach care tube feeds/TPN IV abx nurse care
