Dilated and restrictive cardiomyopathy Flashcards
Pathophysiology of dilated cardiomyopathy
Causative agents weaken myocardial contractions -> compensatory remodeling and hypertrophy (eccentric) -> weak myocardial contractility and systolic dysfunction-> reduced EF and HF
Etiology of dilated cardiomyopathy
Idiopathic, familial
Alcohol and cocaine
Peripartum onset
Viral (coxsackie B, Chagas, HIV)
Cardio toxic drugs (trastuzumsb)
SLE, amyloid
Symptoms, diagnosis, and treatment of dilated cardiomyopathy
Dyspnea, angina, HF
Diagnosis:
Echo: dilated ventricle
X-ray: cardiomegaly
Treatment: treat HF, stop offending agents, consider ICD/resynchronization therapy or heart transplant
Pathophysiology of restrictive cardiomyopathy
Infiltration of myocardium -> stiff ventricles and diastolic dysfunction
Etiology of restrictive
Amyloid, sarcoid, hemochromatosis, endomyocardial fibrosis, radiation
Diagnosis and treatment of restrictive
Echo, ecg, X-ray all show nonspecific changes
Treatment is symptomatic control, poor prognosis
