Parencyhma (Capillary endothelium. Alveolar epithelium and BM)
Insterstitium (perivascular and perilymphatics)
Mc cause of diffuse lung disease
Interstitial pulmonary fibrosis
What are the risk factors of diffuse lung diseases?
Males
60s
Smoker
Positive family
Viral
What is the etiology of DLD?
Activation of fibroblast leading to fibrosis
What is the CP triad of DLD?
Gradual Dyspnea >6m
Chronic dry cough
Clubbing
What are the complications?
-Pneumonia Bronchiactsis pneumothorax
- bronchogenic carcinoma
-Corpulmonale and pulmonary embolism
What is the treatment of DLD?
Patient education (smoking cessation, vaccination)
Lung transplantation
Drugs:
Nintendenab (TKI)
Pirfindone (antifibrotics)
Acetyl cysteine (antifirbotic)
Prednisone (CS)600mg X3d
Azathioprine (CS)
Outline the investigations of DLD?
I -> symmetrical chest/ bilateral limited movement
Per-> Dullness
Palp-> TVF equally increased on both sides and centralized trachea
Ausculation: harsh vesicular with decreased airway entry bilaterally and fine crepitation
What are the investigations and results of DLD
Spirometry:
• FEV1 less
• FVC less
• TLC less
• FEV1/FVC more than 0.7
HRCT to differentiate between obstructive and restrictive lung disease
Functional state: 6 minute walking test
Echo: corpulmonale and pulmonary HTN
Biopsy: honeycombing
BAL: lymphocytosis more than 30% exclude
DLco: impaired
